Overview
- Behavioral Health Counseling: provide motivational interviewing (MI) and SUDs counseling relating to various behavioral health needs (i.e., mental health, substance use, complex psychosocial needs, grief support, mandatory reporting, trauma); performs triage, warm hand-offs, risk assessment, short-term psychotherapeutic counseling and/or crisis intervention, ED/hospitalization discharge outreach, and care management services. This position will be required to conduct assessments at home, offsite and/or in community.
- Performs Psychosocial Needs Assessments: conducts biopsychosocial needs assessments and GPRA to help determine appropriate level(s) of care/intervention(s). Assessment includes, but not limited to: adjustment to medical or health condition; care coordination; mental health status; adjustment/compliance to treatment process; substance usage; child and elder abuse/neglect assessments; domestic violence issues; cognitive status (including potential guardian issues). Work with patient and available support systems to develop appropriate treatment plans/meet goals of patient-identified care.
- Coordination of services: share information and coordinate referrals with OHSU and community partners.
- Maintains accurate and timely documentation of patient care activities in an efficient manner per departmental policies and consistent with billing practices. Participates in staff meetings and regularly scheduled supervisory sessions. Participates in patient rounds and care/case conferences as appropriate. Complete all OHSU/Joint Commission mandatory education requirements and activities within established timelines. Yearly competencies as identified by department.
- Assists in training medical interns, residents, clinical support staff, and other faculty staff regarding the profession of social work, identification of psychosocial patient adjustment issues, and theory/provision of selected interventions. Provides consults for and collaboration with providers regarding mental health diagnoses, substance use, SDoH, etc. Works within behavioral health team to seek the advice and counsel whenever such consultation is in the best interest of patients.
- Works with OHSU Patient Relations Department in assessing patient grievances and offering supportive recommendations and participates in continuous quality improvement initiatives.
Responsibilities
- Masters of Social Work (MSW)
- Experience providing addiction-specific assessment and treatment using best practices in the field of substance use disorders
Thorough and working knowledge of: mental health and addiction assessment, treatment planning, individual and group counseling, crisis intervention, advocacy and addiction-specific community resources; Able to accurately utilize established criteria for assessing and assigning a substance use diagnosis; Able to diagnose patients with co-occurring issues Advanced knowledge in the principles of motivational interviewing and trauma-informed care Comprehensive understanding of the interrelationship between medical and psychological systems and their impact on health. Experience working with an underserved adult population experiencing mental illness, substance use, complex psychosocial needs, and chronic illness. - Clinical Social Work Associate (CSWA)
- Certified Alcohol & Drug Counselor (CADC 3)
